New Patient Forms:  To expedite your care at the office, please fill out the forms below and bring them along.  New patients will generally spend 2-4 hours at the office, for evaluation, testing and possibly treatment.  If you are a diabetic, please bring along a snack.

Please budget approximately 3 hours for an initial appointment and up to two hours for a follow-up. 

Please avoid wearing perfumes or eye makeup on the day of your visit, especially if any eye treatment is to be considered.

North Office 200 W 103rd St, Carmel, IN 46290, (Indianapolis/Carmel): 

Patients generally seen here on weekdays, except Thursdays.

South Office 555 E County Line Road, Greenwood.  Patients are seen on Thursdays.

The South Office is across the street from the golf course and adjacent to the Greenwood Airport and Urology of Indiana.  It is located in the Indiana American Office Plaza.
